Current Clark County residential and/or mailing address

Statement that you want your voter registration kept confidential per NRS 293.558

Daytime phone number in case we have any questions (optional)

Nevada Voter Registration Number from your Voter Registration Card (optional)

Handwritten signature (if you are physically disabled and cannot sign your name, call the Election Department at 702-455-8683 for instructions)

Current Date
