Individuals from other countries may have bank accounts, pensions or retirement funds in their country of origin. It is frequently necessary for verification that an individual is alive in order to be able to access or receive installments from such bank accounts, retirement funds, and pensions. Verification is normally done utilizing a document or form known as a "Proof of Life," which requires the County Clerk to verify that an individual is alive and that the information being provided is correct. OUR OFFICE DOES NOT PROVIDE THESE DOCUMENTS OR HAVE SUCH FORMS AVAILABLE. Normally, the forms are sent by the respective country of origin.
In order for the County Clerk to sign the document, it must be in English or the English translation must be provided. The customer must be able to provide backup documentation regarding any statement or information on the form that the County Clerk is being asked to verify. For example, to prove residence address, a driver's license, utility bill or other document with the home address must be provided. If the document asks for information regarding a spouse, then backup documentation must be produced for any information regarding the spouse that the County Clerk is being asked to verify, (e.g., date of birth, address, etc.). NOTE: Dates on the form will be written in the English format, (i.e., month, day, then year rather than day, month, then year as is done in Europe). If an individual indicates that their spouse is deceased, a death certificate would need to be provided to verify that fact.
Customers who require the signature of the County Clerk on a Proof of Life document are asked to call to schedule an appointment, because the County Clerk (not one of her deputy clerks) signs these documents herself. The Assistant County Clerk may on occasion sign such documents in absence of the County Clerk. If you need to have a Proof of Life document signed or have questions regarding this process, please call (702) 671-0701 during normal business hours to schedule an appointment. Customers are advised to schedule appointments as soon as possible after receiving notice from their country that such a document is required. There is normally a deadline for submitting such documents, and we cannot be responsible for missed deadlines as a result of waiting too long to schedule an appointment if the County Clerk is not immediately available.
