L&G Instructions
A short letter from the Department of Business License to all privileged business license applicants giving suggestions on filing their applications.
 
Metro Application Memo/Instructions
This packet contains information from the Las Vegas Metropolitan Police Department on completing their required forms (Personal History and Personal Financial Questionnaires), fingerprinting requirements, and alcohol awareness training locations.
 
Nevada Business Registration
The Nevada Business Registration Form (NBR) is the main part of the application and must be completed.   An advantage of this form is that it is also utilized by most state agencies; therefore you do not have to repeat all the pertinent information to each agency.  The NBR has specific directions, please be sure to read it carefully and completely fill out the document. 
 
Affirmation of Compliance (Industrial Insurance) Form
Industrial Insurance requires an Affirmation of Compliance Form, which indicates you have complied with industrial insurance coverage requirements in Nevada (NRS 616).   The form needs to be completed;"signed in the presence of a Business License Technician"; and submitted to the Business License Office prior to the issuance of a business license.   If this form is mailed or faxed to our office, it must have the signature notarized.   If you have questions or need more information regarding this form, contact the State of Nevada, Department of Business &Industry, Division of Industrial Relations, Industrial Insurance Regulation Section , 1301 North Green Valley Parkway, Suite 200, Henderson, NV  89104, (702) 486-9000. , 1301 North Green Valley Parkway, Suite 200, Henderson, NV  89104, (702) 486-9000.
 
Child Support Documents
The Federal Welfare Reform Act was implemented by the 1997 Session of the Nevada Legislature by SB 356. In all cases, Nevada law (NRS 425.520) prohibits the Business License Department from issuing a business license, permit, or certificate unless the owner of the business is in compliance with Child Support laws. Completion of the CHILD SUPPORT COMPLIANCE STATEMENT OR EXEMPTION FORM is required with appropriate signatures and social security numbers.  An additional letter of instruction is included with the attached forms.
 
Liquor Summary
The Department of Business License has summarized the main types of businesses and accompanying liquor licenses available to those businesses in a 2-page handout.
 
Liquor Application
This application is required for all applicants for a liquor license in the unincorporated areas of Clark County (not within city licensing jurisdictions).  If you do not know if your business location is in the jurisdiction of the Clark County Department of Business License, you may refer to the Jurisdiction Locator on the main Business License web page.
 
Gaming Summary
TheDepartment of Business License has summarized the main types of businesses and accompanying gaming licenses available to those businesses in a 2-page handout.
 
GAM/GEN/CON  Applications
This three (3) page application is required for all applicants for a gaming license in the unincorporated areas of Clark County (not within city licensing jurisdictions).  If you do not know if your business location is in the jurisdiction of the Clark County Department of Business License, you may refer to the Jurisdiction Locator on the main Business License web page.
 
Gaming Supplement
This form is required for all applicants filing for a gaming licensein the unincorporated area of Clark County.  NOTE: City gaming license applicants do not need to complete this form.
 
Floor Plan
The Department of Business License requires that all applicants for privileged business licenses submit a detailed floor plan of their proposed business location on an 8 by 11 sheet of paper, detailing all gaming, restaurant, bar/lounge, restroom, and kitchen areas and all physical barriers between any gaming, restaurant, bar/lounge, and kitchen areas.
 
Business License Waiver
Due to the high volume of investigations completed by the Department of Business License and Las Vegas Metropolitan Police Department, we are unable to predict when an applicants determination of suitability will be completed and ready for presentation to the Liquor and Gaming Licensing Board for action.  Nevada state law requires that applicants be notified, in writing, at least 21 working days prior to being included on a public agenda.  In most cases, the Department and Metro are prepared to present cases to the Board under that 21-day window, so we ask that applicants waive that 21-day window so as to better serve their interest and enable us to place their item on the next available agenda.
 
Authorization to Release Information
Each applicant for a privileged business license will be required to complete two (2) original, notarized-signature forms giving authorization to the Las Vegas Metropolitan Police Department to conduct a background investigation.
 
Personal History Questionnaire
Each applicant for a privileged business license will be required to complete a form detailing their personal history, including personal references, legal history, family connections and history, and past residence information.  This form requires notarized signatures in several places. A color, passport-sized photograph of the applicant is required to be attached to this form.

Financial Questionnaire's
Each applicant for a privileged business license will be required to complete a form detailing their financial history, including personal bank accounts, investments and history, and past income tax information.  This form requires notarized signatures in several places.  A copy of the applicants past three (3) years of federal income tax filings and last three (3) months of bank statements required to be attached to this form.
