HOPE (Homeless Outreach Project and Evaluation)/SOAR Program
Purpose
The HOPE Team manages a 4 year Social Security HOPE grant (Homeless Outreach Project and Evaluation) starting 7/04 to assist the eligible disabled chronically homeless through outreach to apply for SSI and Social Security Disability. HOPE can also assist eligible disabled chronically homeless clients who are in suspense or terminated from their SSI/SSD benefits. In the 4th year, CCSS will make this part of the services offered by the agency.

Eligibility
The Client must:
1. Must be a citizen or qualified legal permanent resident of the United States.
2. Must be chronically homeless.*
3. Have not applied for SSI and/or Social Security Disability.
4. Has been suspended or terminated from SSI or Social Security Disability.
Required Documents Include
1. A completed HOPE Outreach Referral Form.
2. A one year disability statement from a Medical Doctor.
3. Medical Records that substantiate the medical/psychiatric problem.
Program Benefits
HOPE clients will receive case management of their HOPE case, including:
1. Assistance with their social security application and follow up requests for information.
2. A replacement id and birth certificate.
3. Transportation to and from and assistance with their Social Security office visits.
4. Transportation to and from Doctor’s appointments.
5. Assistance with their Medicaid & Food Stamp applications.
6. DAS (rental assistance).
7. MAS (medical assistance), with an assigned worker on an appointment basis.
8. Assistance and referrals to other agencies to help meet their needs.

For CCSS workers, complete a HOPE application, as on the intranet, under outreach forms, complete per directions and fax to 385-1943 or thousand mile to Bernard Webber and Patricia Ellis, CCSS at Catholic Charities.

* Homeless definition-homeless for at least once year or have multiple periods of homelessness in a multi-year period, in
places not meant for human habitation, in an emergency shelter, in transitional or supportive housing for homeless
persons who originally came from the streets or emergency shelters, in any of the above places but is spending a short
time in a hospital or other institution, in an institution such as mental health or a substance abuse treatment facility or
jail/prison and is being discharged within the near future, but has no permanent address or resources/support networks to
find housing upon discharge, or living "doubled up" with friends, relatives, etc.
